Step 1
We'll call you to discuss insurance information.

Our admissions department will contact you to gather the information we need.

You'll Need:

  • your social security number
  • insurance information
  • insurance authorization number (if applicable)

Step 2
You'll have an initial prep appointment.

Before your stay, our team will schedule an appointment at the hospital to gather the health information that we need.

Appointment Duration

About 90 Minutes. This may include medical tests ordered by your doctor.

You'll Need:

  • all medicine taken regularly
  • any herbal supplements you may use
  • insurance authorization number (if applicable)

Minors

Minors must be accompanied by a parent or legal guardian.

Insurance Authorization Number

Some insurance providers require authorization for certain hospital services. Upon approval, the insurance company will provide you an authorization number.

Given a Number? Keep It On Hand

If this applies to you, you'll need to provide the number during pre-registration and your pre-admission interview.

Don't Have a Number?

If you don't provide the number, your surgery may be delayed or rescheduled, and any services performed may also become your financial responsibility.
